What Do I Do to get a Card?

What do I need to bring? It is MANDATORY for enrollment that TWO forms of identification documentation are brought to the enrollment. One document must be a government-issued photo ID and one must confirm that the applicant is a U.S. Citizen, a U.S. National, or a lawful Permanent Resident of the U.S. If a single document meets all of these requirements, a second document from the list is still required.
